You can download and complete the PHI disclosure authorization form from the hospital where you received care and return it via mail, email, fax, or in person. You can also request a form from the hospital or make the request in a letter addressed to the hospital's Health Information Management (HIM) department. Make sure to include a copy of your photo identification (i.e., driver's license).Some of the patient's test results are available on Health Records Online.
The complete histories of deceased patients or the complete historical medical records of current or former patients are available through a request addressed to the HIM department of one of our hospitals. The patient or their legal representative must complete, sign and date the form in its entirety. Once UMC HIM receives the form, it will take a minimum of 10 business days to process the request. In accordance with Nevada federal and state regulations, you may be charged 60 cents per page for copies of records for personal use or insurance purposes. We accept applications online and by phone. Once we have your information, we'll send the request to your doctor. You can receive printed or digital copies of your medical history. We recommend that you obtain a digital copy, as our secure download process is safer than sending private information by mail.
